Legislators RPG simple survey 1 draft, was Re: [CAR-PGa] Survey of State & Federal congresses?

Greetings again,

The original link was just a rough idea in a blog posting.
The earlier link was just long enough that some email clients wrap it
without the link fully showing up in the browse.

I have now begun to draft the survey more formally.
Here is the web version so far:

I would welcome feedback on this. Once complete, I will create a printed
version for postal mail, and a phone script as well for attempting the
survey via phone call.
